Open enrollment is the one time during the year you can make changes to your benefits without experiencing a qualified life event such as marriage, divorce, or birth of a child.

This year’s enrollment is a passive enrollment

Your current benefits will automatically carry over to the new plan year, unless you make changes. However, you must re-enroll in the health savings account (HSA) or flexible spending accounts (FSA) if you want to fund one in the new plan year.

Eligibility

You are eligible to participate in Generations Healthcare’s benefits program if you are a regular, full-time employee working at least 30 hours per week. If you are a new employee enrolling for the first time, your coverage will be effective on the first of the month following your date of hire, or change of status to full-time. You have up to 30 days from your hire date or the date you become eligible for benefits to enroll. Once your enrollment is processed, your coverage and any paycheck deductions will be retroactive to your health insurance effective date.

Dependent Eligibility

Eligible dependents include:

  • Your legal spouse or registered domestic partner
  • Children up to age 26, including natural children, stepchildren, legally adopted children, children for whom you are the legal guardian, foster children, children for whom you are legally responsible to provide health coverage under a Qualified Medical Child Support Order (QMCSO)
  • Disabled children over age 26 if unmarried, incapable of self-support, dependent on you for primary support and the disability occurred before the age of 26

Benefits Enrollment

The Generations Healthcare benefits plan year is effective on November 1, 2025 through October 31, 2026. Your selection is in force from your benefit eligibility date through October 31st, unless you have a “Qualifying Life Event.” Generations Healthcare conducts an Annual Open Enrollment that will allow you to make changes for the following plan year.

GHC offers an Employee Assistance Program through Mutual of Omaha​. This benefit is available to ALL employees and family members in their household. This is a completely confidential and free program that provides you with access to EAP professionals to help with work/life balance, family and work issues, and referrals for legal and financial assistance.

Telemedicine

Our medical plans include Telehealth services. You can connect with a board-certified doctor via secure video chat or phone to get the care you need for a wide range of minor conditions, without leaving your home or office. Televisits cost less than going to a convenience or urgent care clinic, and significantly less than going to the emergency room.

Remember, you should only use telehealth services for non-life threatening conditions.

What is a Qualifying Life Event (QLE)?

A Qualifying Life Event is a change in your situation — like getting married, having a baby, or losing health coverage — that can make you eligible for a Special Enrollment Period, allowing you to enroll in health insurance outside the yearly Open Enrollment Period.

There are 4 basic types of qualifying life events. (The following are examples, not a full list.)

Loss of health coverage

  • Losing existing health coverage, including job-based, individual, and student plans
  • Losing eligibility for Medicare, Medicaid, or CHIP
  • Turning 26 and losing coverage through a parent’s plan

Family status changes

  • Getting married or divorced
  • Having a baby or adopting a child
  • Death in the family

Change in residence

  • Moving to a different ZIP code or county
  • A student moving to or from the place they attend school

How Can I Save Money with Tax-Free Accounts (FSAs and HSA)?

Keep more of your paycheck by using pre-tax FSA and HSA dollars to pay for eligible expenses.

For 2025, the contribution limits are:

  • Health care FSA: $3,300
  • Dependent care FSA: $5,000
  • HSA for individuals: $4,300
  • HSA for families: $8,550

Contribution limits are set annually by the IRS.

Life and AD&D

Life insurance pays a benefit to your beneficiary if you pass away. Accidental death and dismemberment (AD&D) insurance pays a benefit to you if you’re seriously injured or to your beneficiary if you pass away because of a covered accident.

Disability

An unexpected injury or illness can create a financial burden. Disability insurance replaces a portion of your income when you experience a qualifying disability and are unable to work.

Voluntary Life and AD&D

Enhance your financial protection with voluntary life and AD&D coverage. You can elect increased coverage to help financially protect yourself and your dependents.

Accident

Accident insurance can help with expenses incurred due to an injury, ongoing living expenses, and more. Claims payments are made in flat amounts based on services incurred after an accident.

Hospital Indemnity

Hospital indemnity insurance provides a lump-sum benefit due to a hospitalization. The plan includes benefits for a hospital confinement, critical care, and hospital admission.

Critical Illness

If you are diagnosed with a critical illness, you may need additional financial support to help offset treatment costs and cover day-to-day expenses. The plan pays a lump-sum benefit that you can use to pay your mortgage, seek experimental treatment, or handle unexpected expenses.

Pet Insurance

Let’s not forget about our furry friends! Pet insurance helps cover the cost of your pet’s care including preventive care, accidents, illnesses, X-rays, office visits, medications, surgeries, and hospital stays. Cost for coverage may depend on your pet’s age, breed, and elected plan.
